Return of CAS original documents

We keep original documents on file for up to one year before destroying them. If you would like your original documents to be returned to you, download and fill out a Return Original Documents form and mail or fax it to our office. If you are currently registered for the upcoming Match, you must wait until the Match is concluded in order to request your original documents back. Requests from current Match applicants received prior to the Match results day will not be processed. There is a $20 fee for this service. Documents are mailed to applicants via regular mail to the address indicated on the form. Only non-confidential documents can be returned.
