Original CAS Documents - return requestOur office will return original CAS documents (via regular mail) at the applicant's request. There is a $20 fee for this service. Please complete, print, sign, and return this form to have your documents returned to you. Documents will NOT be returned until AFTER the match takes place. All requests must be in writing; we cannot help you over the phone. Please note that our office will store your originals documents for one year after the Match is concluded. Confidential documents will not be returned. Please note that 2007 was SF Match's first year distributing applications to Plastic Surgery residency programs. We do not have files or documents from previous years. Please contact the former distribution service for Plastic Surgery, Maricopa County Central Application Service to inquiry about your old application/documents.
