Distribution of CAS applications to Programs

The Distribution Form is used to specify which training programs shall receive your application materials. Login to the online directory to view the list of available programs and list them on your distribution form using their 5-digit program number and name.

The Initial Distribution Form should be used if you are selecting your programs for distribution for the first time. Additional programs can be added later using the Supplemental Distribution Form.

All Ophthalmology Fellowship subspecialties accept CAS except for Neuro-Ophthalmology and Pathology. You may distribute up to 8 programs at no cost on your Initial Distribution. A $35 fee applies for each additional or supplemental program added after initial distribution.
